The letters of recommendation called for in Part III should be submitted directly by the letter writers.
Does your high school offer Advanced Placement courses? YesNo
Does your high school offer Advanced Placement calculus? YesNo
List all Advanced Placement courses and mathematics courses at the level of calculus and above (give semester and year when taken). Also give the grade in the course, year of AP exam taken, and AP exam score
Please arrange for one or two letters of recommendations to be submitted on your behalf. At least one of these should be from a math teacher who has had at least the equivalent of one semester of class contact with you. You might want to ask someone to write a letter who can make a reasonably detailed assessment of your abilities, motivation, strengths, and weaknesses.
Please list the names, position (e.g. math teacher at Elk Rapids High School) and contact information for the people who will be writing letters of recommendation.
Each letter should be submitted directly by the letter writer. However, it is the applicant’s responsibility both to provide the letter writer with all relevant information and to remind the letter writer to submit the letter on time.
FERPA ACT OF 1974 AND ACCESS TO LETTERS OF RECOMMENDATION
Under the provisions of the Family Educational Rights and Privacy Act of 1974, you will have access to the information provided in letters of recommendation unless you have waived such access. Please select the appropriate box and enter the date below to inform us of your decision. Your choice will not affect your eligibility for any of the scholarships.
While there is no preferred format, a letter of recommendation should address the applicants character, capacity, motivation, initiative, helpfulness to the High School academic life, activities inside and/or outside of academics and demonstration of interest in Mathematics. In particular, it will be helpful if the letter addresses the ability of the student in Mathematics, indicating both strengths and weaknesses, and the potential for growth.
Letters of recommendation should be submitted directly by the letter writer, and not through the applicant. The contents of letters of recommendation will be kept as confidential as current and future laws regarding confidentiality allow.
